excellent stuff -- you know maybe not , that we produce audit & practice management software here --constantly evolving & free to try & use http://www.crystalsolutions.org

your approach to audit  is very interesting, as i am setting up audit for one of the colleges in the new year, and just starting to organise my software to reflect their custom needs - i could get out a version for you, martin perhaps if you want.......

my software  hopefully makes inputing  info much easier  than a spreadsheet
(data can be exported back to the spreadsheet or imported from the spreadsheet if required) and there is a report system already in place. the software can be customised as required, and works as a standalone........

we (both val and i are o'paths) typically get our patients into the program at the end of the day in around 5-10 mins depending on the detail we are putting in - and that includes the accounts!!

have a look at the s'ware - runs on mac and windows and allows for 50 patients while you try it -----

it is constantly evolving and one of the areas where i need to do work is in the order/logic of the first 2,3 & 4 pages ------- which at present are a little haphazard -- i was hoping to include what i am learning from the college clinic contracts --- but these clinic outcome documents are exactly what i was looking for, as they are "full audit" with patient response....

i look forward to speaking with you about all this martin..

good work

ive attached your documents above as word.doc --and here below as pdf  for non word owners, or for us mac users (tho' pages opens .doc)
